Performance Plan Template

While working to develop our team, sometimes it is necessary to correct or improve an individual’s behavior or performance. This plan serves as documentation if the problem continues. In consultation with Human Resources, develop a corrective plan for performance improvement. This plan may also be used prior to a D.E.S.K. conversation in hopes of improving performance. What steps need to be taken by you as the leader, the employee, and the organization? Be specific about the consequences if the behavior continues and the desired time frame in which action must be taken. If the skill or behavior improves, reinforce the improved performance and reassure the employee that you appreciate their work on this skill.
